Definition
Comment on why the case is excluded from the data transmission process. In this field, you can/should store a text specifying why this case has been excluded.
You do not have to specify the person who created the entry. Each entry is stored in the system with a time stamp.
When entries are changed, it can, however, be useful to enter the name of the person who made the changes. Only the last person to have made changes is logged.
Example
"Agreed by phone with Mr. Smith of MEDISHIELD on 02/01/98".
"Letter from GELIFE on 03/02/98".
