Specifies the format of a form. Each form is assigned a form type.

Procedure
Examples
In order to scan a completed questionnaire, the form type FSCAN must
already be assigned to the relevant form. Forms that are assigned the form type FSCAN have code numbers printed beside each question. When the questionnaire is scanned, the system reads these code numbers and assigns them to the appropriate fields.
