The keyword search is a further means of coding diagnoses that is supported in the IS-H System.

Within your institution, you can set up keyword catalogs (multiaxial catalogs) for coding diagnoses.

You can create catalogs of keywords, and set up hierarchies within these keyword catalogs. Entries from a diagnosis coding catalog are assigned to each of these keywords.

You proceed as follows when coding diagnoses using keyword catalogs:

The selection screen of the keyword search displays a maximum of two hierarchically structured keyword catalogs. You can adjust the scope of the hierarchic display by opening and closing keywords.

You select the required keywords by means of a simple mouse-click. The system returns a dialog box listing the diagnoses that are assigned to the keywords. From this list, you can select the diagnoses you want to code.

Setting Up Catalogs

You are required to work through a number of activities to set up the catalogs for the keyword search:

  1. Diagnoses - define catalog categories
    In this step, you store the catalog categories for the keyword search.
  2. Diagnoses - maintain codes
    In this step, you store the keywords for the catalog categories you defined in the previous step.
    You need to set the Generic header flag for all keyword entries, since these are not suitable for diagnosis coding.
    By setting the Diagnosis group flag, you specify whether the diagnoses are to be subsequently classified under, or assigned to the keyword in question. This means that you need to set the group indicator for all keyword entries.
  3. Define diagnosis assignment types for hierarchy creation
    You need to specify two diagnosis assignment types for each keyword catalog.
    1. Specify a hierarchical structure for the keyword catalog.
    2. Assign the keyword catalog to a specific diagnosis coding catalog which contains suitable coded entries for diagnosis coding.
  4. Maintain hierarchic diagnosis catalog
    1. Set up the hierarchy for a keyword catalog.
    2. Assign diagnosis codes to a keyword catalog.

Example

You want to set up the keyword catalogs

  • BA body areas
  • IT injury types

For this, you define the catalog categories BA and IT.

You enter the keywords for the catalog categories

  • BA body areas such as arm, leg, foot, etc. and
  • IT injury types such as contusion, sprain, etc.

as diagnosis codes for BA or IT in the diagnosis coding catalog.
You need to set the Group diagnosis and Generic header indicators for these entries in the diagnosis coding catalog.

You now define two diagnosis assignment types for each keyword catalog.

The first of these assignment types defines the assignment within the catalog (BA->BA or IT->IT), and furthermore enables a hierarchy to be set up for keywords. Here you can subordinate the entry foot to the entry leg.

The second assignment type defines the assignment of diagnosis codes, e.g. from the ICD-9 catalog to keywords (BA->ICD-9 or IT->ICD-9).

You could make the following assignment of diagnoses to keywords, for example:

You assign the diagnoses:

  • 841.9 spraon/sprain: elbow and lower arm, unspecified
  • 844.0 colateral ligament of the knee

to the keyword sprains.

You apply this procedure for all keywords.

Requirements

To be able to use an OU-related keyword catalog, you have to define keyword catalogs for OU-related keyword search for the OUs in question.

Recommendation

Where the scope of keyword catalogs is extensive, a hierarchical subdivision simplififies the diagnosis coding process.

Activities

You create keyword catalogs by defining diagnosis coding catalog categories and entering keywords for the diagnosis coding catalog categories in the diagnosis coding catalog.

You need to set the Group diagnosis and Generic header indicators for the keywords in the diagnosis coding catalog.

You have to specify two diagnosis assignment types for each keyword catalog.

If all these requirements are satisfied, you can set up a hierarchy for the keyword catalog, then assign diagnoses to the keywords.

Further notes

The keyword catalogs are available for the OU-related keyword search once you have made the corresponding settings for the departmental OE in Customizing.

By entering the parameter value SKAT for the user parameter DKE, you specify that the keyword catalog is always displayed with the Possible entries function for coding diagnoses in diagnosis processing.
