Use
In this IMG activity, you define how Medical Record Management is to be integrated into your hospital or department organization. These settings can be made with regard to:
- An institution
- An institution and an organizational unit (OU)
You make these settings for Medical Record Management on two levels.
- On the level OU-Related Parameters of Medical Record Mgmt, you make basic specifications for the medical record organization of an OU.
- On the level Other Medical Records to Be Borrowed, you can define medical records that are also to be borrowed under the conditions specified here, but which do not correspond to the medical record organization form specified on the first level.
Activities
OU-Related Parameters of Medical Record Management
If the parameters are to be valid throughout your institution, enter an asterisk (*) in the Org. Unit field. If you specify an OU from the hierarchy, the parameters stored for this entry apply to all organizational units below this OU in the OU hierarchy.
In the Document Type ID field, specify the document type for the medical record you created. Number ranges are assigned to the document type for the assignment of medical record numbers. The document type ID is also used as a default value for the descriptive text of the medical record when it is created.
In the Archive field, you specify a medical record archive in which the medical records are kept. The system proposes the archive you specify here as the medical record archive when a medical record is created. You can only enter OUs for which the Medical Record Archive checkbox is flagged in the master record.
You use the organization forms of Medical Record Management to specify for which events medical records are to be created for inpatients and outpatients. Here you can specify the same or different organization forms for inpatients and outpatients.
The IS-H System supports the following organization forms:
- Create Medical Record per Patient
- Create Medical Record per Patient and Clinic
Here "clinic' refers to an OU that is located above a department in the organizational hierarchy.
- Create Medical Record per Patient and Department
- Create Medical Record per Patient and Treatment OU
- Create Medical Record per Case
- Create Medical Record per Case and Department
- Create Medical Record per Case and Treatment OU
In the Temporary Medical Record group box, you can specify if, and under which conditions, a temporary medical record is to be created. The following variants are supported in the system:
- Do not Create Temporary Medical Record
- Always Create Temporary Medical Record
- Create Temporary medical Record if Patient Medical Record Exists
The temporary medical record should satisfy the following requirements of the medical record organization:
- The decision as to whether and according to which organization form a medical record is to be created is not to be made until the patient is discharged. The medical documents pertaining to the patient's stay are kept in the temporary medical record and once this record is returned to the archive it is decided whether the documents must be appended to an existing medical record, or whether a new record must be created.
- It should be possible for an existing medical record that was borrowed for the patient's treatment to be returned immediately after this treatment and the medical documents for the current treatment to be subsequently supplied to the archive via the temporary medical record.
The system automatically creates a temporary medical record on a case basis (organization form 3) when a patient is admitted, transferred or comes for an outpatient visit. However, this is only possible for movements with the status Actual. After the temporary medical record has been returned and its contents have been appended to a patient medical record, it is deleted.
Note: With regard to outpatient visits, a temporary medical record is only created for the first visit. If the temporary medical record is deleted after the first visit, additional temporary medical records are not created for further visits of the case in the same OU. However, if the case has a further visit in a different OU, the system creates a temporary medical record providing the OU is parameterized accordingly.
For the creation of medical records, the SAP System supports the following operative procedures in accordance with the parameters set for the admitting OU:
- Create at Admission/Transfer
If, in accordance with the defined organization form, a medical record does not exist when a patient is admitted, transferred, or attends the clinic for an outpatient visit, the system displays a dialog box for you to specify whether a medical record is to be created.
- Auto. Create at Adm./Trans.
If, in accordance with the defined organization form, a medical record does not exist when a patient is admitted, transferred, or attends the clinic for an outpatient visit, the system creates a medical record automatically.
- Create via Movement List
If, in accordance with the defined organization form, a medical record does not exist when a patient is admitted, transferred, or attends the clinic for an outpatient visit, the system records this to enable the subsequent creation of a medical record. You then create medical records at a later time using a list of admissions or transfers that have taken place.
- With Simultaneous Borrowing
A further parameter lets you specify whether, once created, a medical record is to be automatically lent to the admitting organizational unit. In the process, a borrowing record is created with
- Borrowing reason 1 (patient treatment)
- Borrower category organizational unit
- Admitting organizational unit as the borrower
Note 1: If you are using temporary medical record organization 1, the system ignores the settings made under Creation of Medical Records.
Note 2: If you want, for example, the system to automatically create a medical record for the mother of a healthy newborn without creating a medical record for the newborn, you can use the admission type in the enhancement NKAV0003 to prevent the automatic creation of a medical record for the newborn. For more information about how to implement enhancements, refer to the system documentation or the online documentation on the transaction CMOD (Project Management of SAP Enhancements).
For the borrowing of medical records, the system supports the following operative procedures in accordance with the parameters set for the admitting OU:
- Auto. Borrow at Admiss./Transfer
If, in accordance with the defined organization form, a medical record already exists when a patient is admitted, transferred, or attends the clinic for an outpatient visit, this can be automatically lent to the admitting organizational unit (in the background). The system creates a borrowing record with borrowing reason 1 (patient treatment), borrower category organizational unit and the admitting organizational unit as the borrower.
- Auto. Request at Admiss/Trans.
If, in accordance with the defined organization form, a medical record already exists when a patient is admitted, transferred, or attends the clinic for an outpatient visit, a borrowing request can be automatically created in the background. The system creates a request with borrowing reason 1 (patient treatment), borrower category organizational unit and the admitting organizational unit as the requester. The requested medical records can then be checked out of the archive and assigned the status borrowed via a list of requests that have been made.
- Borrow via Movement List
If, in accordance with the defined organization form, a medical record already exists when a patient is admitted, transferred, or attends the clinic for an outpatient visit, the system records this to enable the subsequent borrowing of a medical record. You then borrow medical records at a later date via a list of admissions/transfers that have taken place.
Note 1: If a medical record is flagged as microfilmed, the system ignores setting Auto. Borrow at Admiss./Trans.
Note 2: Use the option Auto. Borrow at Admiss./Trans. only if the medical record is to be requested once for an inpatient hospital stay or for an outpatient treatment. If, by contrast, the medical record is to be requested several times over for a number
