Documentation

In this section of Magnetic Media Filing, you specify both the source of Relevé slips being used in your submission and, if applicable, your Quebec enterprise number (NEQ). For each submission, select one of the following four radio buttons.

• Ministère du Revenu du Québec
  Select this option if the slips were obtained from le Ministère du Revenu du Québec.
• Third party
  Select this option if the slips are facsimiles obtained from a third party.
• Transmitter
  Select this option if the slips are facsimiles produced by the transmitter.
• Combination of above
  Select this option if the source of the Relevé slips is any combination of the prior three options.

Once you have specified the source of Relevé slips, enter your Quebec enterprise number (NEQ), if applicable. This is the number assigned to your enterprise by the Inspector General of Financial Institutions or the clerk of the Superior Court. If no such number has been assigned to your enterprise, then leave this field blank.
