Use
Enter in this field the date that you receive notice of intention to start the PPP or APP from an employee .
- In the case of Statutory Paternity Pay (birth or adoption), the employee must provide notice by submitting a self-certificate to the employer. - Statutory Paternity Pay (adoption) - Self-certificate form SC4
- Statutory Paternity Pay (birth) - Self-certificate form SC3
- In the case of Statutory Adoption Pay, a Matching Certificate must be submitted to the employer within one week of issue of this certificate.
