Diagnosis
You have tried to upload a document, but you are not authorized in the project or its folder.
System Response
The document cannot be uploaded.
Procedure
You can see which authorizations you are missing, under System -> Utilities -> Check Display Auth.. Ask the person responsible, for these authorizations.
