Indicate whether there is a change in address of the:
- Employer since the tax was last filed
- Person Responsible since the tax was last filed. The Person Responsible is the same person whose Personnel Number you have entered as the Personnel No. of Person Resp.
This information is printed as a Y (Yes) or an N (No) on the e-file for Form 24.
