Specific activities within the Medication module require the entry of a reason. These reasons are assigned to the corresponding activities in code form.

Example
Before they can be used, the entries in the formulary must be activated. If an entry is deactivated, a reason must be entered.
When a drug is ordered a reason can be entered (therapeutic, prophylactic, empiric).
A reason must be entered if there is a time or quantity difference between ordered and actually administered drugs.
If an ordered drug is not administered a reason must be entered.
