- 1 - Employee hiring in first employment (with record)
- 2 - Employee hiring with previous employment
- 3 - Employee transfer from one branch office of the same company, or other, with costs to the transferor.
- 4 - Employee transfer from one branch office of the same company, or other, without costs to the transferor.
