For each Jamsostek office that your company reports to, you must provide some detailed information that is relevant to your company. These information includes:

• the Jamsostek identification number for the company, or subsidiary that reports to that Jamsostek office. Each employee will be given an identification number which consists of two sets of numbers. The second set of numbers corresponds to this field.
• the Jamsostek office that the company or subsidiary reports to.
• the health insurance scheme that the company or subsidiary provides the employees. The company or subsidiary can provide its own health insurance scheme to its employees if it provides better coverage than the Jamsostek health care benefit. In this case, the company or subsidiary is allowed not to pay this part of Jamsostek.
• the Jamsostek accident insurance contribution level provided. This depends on the type of work employed in the company or subsidiary.
