Use
This step enables you to indicate after how many calendar days the 42nd-week, 13th-week and the 8th-month sickness notification should be prepared for the Employee Insurance Agency (UWV). The implementing body should receive this notification no later than 42 weeks, 13 weeks or 8 months.
The determination of which of the two notifications is relevant depends on whether you are the own risk bearer for the Partially Disabled Workers Resumption of Work Act and/or the Invalidity Insurance Act. If you are not the own risk bearer, the 13th-week notification applies; otherwise the 8th-month notification of sickness applies.
Example
The Employee Insurance Agency 42nd-week notification of sickness should be prepared after 42 weeks (i.e. 42 weeks x 7 days = 294 calendar days)
The Employee Insurance Agency 13th-week notification of sickness should be prepared after 12 weeks (i.e. 12 weeks x 7 days = 84 calendar days). be prepared after 8 months (i.e. 7.5 months x 30 days = 225 calendar days).
Requirements
The employee is not entitled to a sickness benefit.
