  • Notification of sickness in connection with a benefit application under the Sickness Benefits Act (ZW)(HR_NL_ZA)

  • Notification of partially disabled resumption of work and other changes by employer - After notification of sickness in connection with a benefit application under the Sickness Benefits Act (ZW) or after a Work and Care Act (WAZO) benefit application (HR_NL_GW)
  • Notification of full recovery - After notification of sickness before 13th week (or 8th month) (HR_NL_VH_13_8)
  • Notification of sickness to Health and Safety Authority (HR_NL_VERZ_ARBO)
  • Notification of recovery to Health and Safety Authority (HR_NL_HERV_ARBO)

The forms' mapping indicates which data should/should not be entered and where the data comes from. The mapping can be found on the HRNL site.

Activities

  1. If you want to modify the form, copy it in the manner described in the Forms section (HRNL_ILL_030).
  2. Make the required modifications or create a whole new layout.
    The SAPscript output uses the SAPscript notification of sickness print structure (P05_S_ZKM), which you can also use for your own form. Some PDF forms are available as well, which have the same technica names as the above-mentioned SAPscript forms.
