Diagnosis

There are two possible causes of error:

You are already processing the patient's vital sign in a different transaction.

You have changed the patient's vital sign but have not yet updated. In the time between the change and the update, you cannot access this vital sign.

Procedure

In the first case you should check whether you are processing the patient's vital sign in another transaction, in the second case you should try to process the patient's vital sign later.
